Most of us have encountered something hot that’s burned or scalded the inside of our mouth—not a pleasant feeling. But what if you have a similar burning sensation without eating or drinking anything to cause it?
It’s not your imagination: It could be a condition called burning mouth syndrome (BMS), the feeling your mouth is burned or scalded without an apparent cause. It’s often accompanied by dryness, numbness, or tingling. You may feel it throughout the mouth, or just in “hot spots” around the lips, tongue or other mouth structures.
Researchers haven’t pinpointed exact causes yet for BMS. It’s most common in women around menopause, connecting it to a possible hormonal imbalance. It’s also been linked to diabetes, nutritional deficiencies, medication, acid reflux, cancer treatment or psychological issues. Because it can persist for years, BMS can contribute to irritability, anxiety or depression.
If you’re experiencing BMS, there are things you can do to diminish its effect. First, though, have your dentist give you a complete oral exam and take a thorough medical history. They can then give you specific treatment recommendations based on what they reveal.
For example, if symptoms seem to increase after brushing your teeth, you might be having a reaction to a toothpaste ingredient, usually the foaming agent sodium lauryl sulfate. Your dentist may recommend experimenting with other toothpaste brands.
Other treatment options include:
- Alleviating dry mouth symptoms by changing medications (as your doctor advises), drinking more water and using saliva-boosting products;
- Quitting smoking and reducing your consumption of alcohol, coffee and spicy foods;
- Chronicling your diet to look for connections between individual foods and BMS flare-ups—you may need to restrict these in your diet.
- And because it seems to aggravate BMS symptoms, reducing acute stress with relaxation techniques or therapeutic counseling.
If your dentist can’t fully diagnose your condition or the steps you take aren’t reducing your symptoms, you may be referred to an oral pathologist (a dental specialist in mouth diseases). The key is not to give up until you find a workable treatment strategy. Through a little trial and error, you may be able to overcome the discomfort of BMS.

The market for sugar alternatives has grown exponentially since saccharin was accidentally discovered in 1878. Today, saccharin has been joined by other FDA-approved zero-calorie artificial sweeteners, including aspartame (“Equal®” or “NutraSweet®”), sucralose (“Splenda®”) and rebaudioside A, derived from the stevia plant. You can also choose low-calorie alcohol sugars like erythritol or xylitol.
With rare exceptions, all these choices are widely considered safe substitutes for table sugar, high fructose corn syrup or other versions of this plentiful carbohydrate. Finding substitutes for sugar is a worthy health goal: besides its role in obesity, sugar is considered a contributing factor in cardiovascular disease and diabetes.
It's also a prime food for oral bacteria that cause dental disease. As bacteria consume sugar they produce acid as a byproduct. Acid softens and dissolves the mineral content in enamel, leading to erosion and the formation of cavities. While saliva normally neutralizes acid after we eat, constant snacking and higher quantities of sugar in our food make it difficult for it to control or neutralize acid in the oral environment.
Because most of us are hard-wired with a “sweet tooth,” it's difficult for many to cut back on sugar. Artificial sweeteners help reduce the amount of sugar in the diet with obvious benefits for general health. It can also make a big difference in your dental health by helping you prevent tooth decay.
One alcohol sugar may even go a step further. In addition to reducing the presence of sugar in the mouth, xylitol (found in chewing gums, candy and breath mints) also seems to reduce bacterial growth by interfering with their ability to ferment the sugar.
If you're considering using an artificial sweetener, get to know them first: some like aspartame aren't suitable for baked goods or cooking, while saccharine or sucralose are. People with a rare genetic condition called phenylketonuria also can't properly process aspartame in the body.

August is National Wellness Month. Since part of staying in good overall health is taking care of your dental health, it's a good time to look at ways you can improve and maintain your oral health. Here are some tips:
Practice good oral hygiene. A fundamental key to a long life of healthy teeth and gums is keeping them clean of dental plaque. This thin biofilm of bacteria and food particles is the number one cause of tooth decay and periodontal (gum) disease. Brushing twice and flossing once each day gets rid of that unpleasant grittiness and reduces your risk of disease.
See your dentist regularly. A good daily oral hygiene habit works best at controlling soft plaque. But any that you miss—a possibility even with great brushing and flossing skill—can harden into calculus (tartar). To remove it, you'll need professional cleaning by a dental professional. The American Dental Association recommends a comprehensive dental cleaning at least twice a year to fully minimize your disease risk.
Eat a low-sugar, dental-friendly diet. Oral bacteria love to feast on the leftovers from your eating, especially sugar. So, cutting back on foods with added sugar isn't just good for other aspects of your health, it can also help "starve out" bacteria and reduce their population in your mouth. You can also boost oral health by eating foods rich in minerals like calcium to maintain strong bones and teeth, and antioxidants that guard against oral cancer.
See your dentist at the first sign of problems. While hygiene, dental care and a nutritious diet can greatly reduce your risk of disease, it won't eliminate it completely. So see your dentist promptly if you notice red, swollen or bleeding gums, mouth pain or unusual spots on your teeth. The sooner you're diagnosed and treated, the less damage from dental disease and future treatment expense you'll endure.
Manage other inflammatory conditions. If you're dealing with a condition like heart disease, diabetes or arthritis, it could increase your risk of gum disease or make any occurrence of it worse. That's because gum disease and many systemic conditions share chronic inflammation as a common link. If an inflammatory condition is not managed through proper treatment, it could worsen any gum disease symptoms you have.
Pursuing wellness is a worthy goal—just be sure you include your oral health in the mix. A healthy mouth is a key ingredient for a healthy life. Although dental care has made incredible advances over the last century, the underlying approach to treating tooth decay has changed little. Today’s dentists treat a decayed tooth in much the same way as their counterparts from the early 20th Century: remove all decayed structure, prepare the tooth and fill the cavity.
Dentists still use that approach not only because of its effectiveness, but also because no other alternative has emerged to match it. But that may change in the not-too-distant future according to recent research.
A research team at Kings College, London has found that a drug called Tideglusib, used for treating Alzheimer’s disease, appears to also stimulate teeth to regrow some of its structure. The drug seemed to cause stem cells to produce dentin, one of the tooth’s main structural layers.
During experimentation, the researchers drilled holes in mouse teeth. They then placed within the holes tiny sponges soaked with Tideglusib. They found that within a matter of weeks the holes had filled with dentin produced by the teeth themselves.
Dentin regeneration isn’t a new phenomenon, but other occurrences of regrowth have only produced it in tiny amounts. The Kings College research, though, gives rise to the hope that stem cell stimulation could produce dentin on a much larger scale. If that proves out, our teeth may be able to create restorations by “filling themselves” that are much more durable and with possibly fewer complications.
As with any medical breakthrough, the practical application for this new discovery may be several years away. But because the medication responsible for dentin regeneration in these experiments with mouse teeth is already available and in use, the process toward an application with dental patients could be relatively short.
If so, a new biological approach to treating tooth decay may one day replace the time-tested filling method we currently use. One day, you won’t need a filling from a dentist—your teeth may do it for you.
While most tooth loss stems from dental disease or injury, another major cause is a condition known as cracked tooth syndrome. What begins as a microscopic crack in an individual tooth’s enamel could ultimately grow to a fracture that endangers its survival.
Most often related to age-related brittleness, expansion and contraction of the enamel surface because of hot foods followed by cold foods and beverages, or grinding habits, cracked tooth syndrome usually occurs in three phases. The first phase is the emergence of miniscule cracks in the outer enamel known as craze lines. These can be very difficult to detect even with x-rays, and usually calls for specialized detection methods such as probing with a sharp instrument (an explorer) or fiber-optic lighting with dye staining to highlight enamel abnormalities. If you have pain symptoms, we may ask you to bite down on a bite stick or rubber pad to locate the area by replicating the sensation.
In the next phase, the craze line grows into a crack that penetrates below the enamel into the tooth’s dentin. Pain becomes more prominent and the risk of infection increases. Left untreated, the crack may enter the third phase, a full break (fracture) occurring deep within the inner layers of the tooth. The deeper the fracture occurs, the more serious the danger to the tooth, especially if the pulp is exposed.
The best treatment approach is to attempt to detect and treat a crack as early as possible. Craze lines and moderate cracks can usually be repaired with restorative materials like composite resins. A deeper crack extending into the pulp may require a root canal treatment and the tooth covered with a permanent, protective crown.
If, however, the fracture is too deep, the tooth may be beyond repair and will need to be extracted and replaced with a dental implant or permanent bridge. In any event, the sooner a cracked tooth is discovered and treated, the greater your chance of avoiding pain, discomfort, and, ultimately, tooth loss.
